You’re invited to be part of an innovative training session on Personal Experience Outcomes and Wisconsin Long-Term Care Options – Family Care, Partnership, and IRIS

As Wisconsin moves toward full implementation of Family Care, Family Care Partnership, and IRIS – three new ways of managing services and funding for the elderly and people with disabilities – one thing is certain: decisions about what services a person gets will be based on how he or she (or their guardians) will answer basic questions about 12 ?personal experience outcome areas,  including:

  • Living arrangements
  • Stability
  • Daily activities
  • Respect and fair treatment
  • Relationships
  • Privacy
  • Community involvement
  • Health and safety

MARC-South, 901 Post Road, Madison (just south of the Beltline, off of Fish Hatchery Rd)

When?

Wednesday, Dec. 9, 6:30 pm – 9 pm Introduction to Wisconsin Long-Term Care Options
Saturday, Dec. 12, 9:30 am - 4:30 pm Using Outcomes to develop service plans — includes lunch
Thursday, Dec. 17, 6 pm – 8 pm Handling disagreements within Family Care — includes dinner

Cost: Free of charge. The costs for curriculum development and this course are covered by a grant from Pathways to Independence, a part of the WI Department of Health Services. Your only obligation is to help us critique and improve this session to ready it for possible state rollout.

Other details: Participants must attend all three sessions. Maximum of 30 attendees. First come, first served.
