Self-Directed Supports Network Training

Join us for our July Network Event - "I Choose Where and With Whom I Live!"

Self-Directed Supports (SDS) within Family Care, IRIS and CIP are now supporting more than 6,000 Wisconsin elders and citizens with disabilities and growing every day! The SDS Network is designed to connect us statewide, regionally and locally. Those connections will enable us to share and learn from everyone’s expertise and experience, to support one another, and to inform and assist others who will receive support and provide support through SDS.

Our focus for this Network Forum is on Wisconsin’s first long-term care outcome, “I Choose Where and With Whom I Live.” There is little in life more important to any of us than our home and what it means to us as the center from which our life flows.

Come and learn how SDS can help in enabling people to have a home in which they are at the center, own or lease where they live, choose who lives with them, and obtain, create or purchase needed support from others.
