Citizen's Guide to Family Care

The Survival Coalition of Wisconsin Disability Organizations has been holding a series of trainings on Family Care around the state.  The next one will be in Racine.

Where: Roma Lodge, 7130 Spring Street, Racine

When: 9:00 a.m. –  4:00 p.m. (Lunch provided), Wednesday, April 29, 2009, also an evening session is planned for 5:30-7:00 p.m. (Refreshments
provided)

What you will learn: An overview of Family Care (Managed Care,
Partnership) and SDS Waiver (IRIS).  How individual funding and services are determined.  What your rights and protections are under Family Care.
How you can make your own decisions under the Self-Directed Supports waiver and much more.

Who should attend: Consumers, family members, service providers, case managers, and others who want to learn more about Family Care.
