The Children’s Hospital of Wisconsin and the Southeast Regional Center for Children and Youth with Special Health Care Needs will sponsor Care in the Community: Making Connections. This conference is for families, physicians, social workers, teachers, and others who care for children and youth with special health care needs. It provides information, resources, and strategies that help them work together to give children and youth with special health care needs coordinated, family-centered care in the community. The conference will be held Wednesday, September 21, 2011 at the Radisson Hotel - Milwaukee West in Wauwatosa, WI from 8:00 a.m. – 3:30 p.m.
The keynote speaker is Mark Sweet, a trainer and consultant with Disability Rights Wisconsin. Mark’s keynote will be Creating Safety: Understanding Her/His Point of View. In addition, Mark will do a breakout session on Positives Outcomes. This presentation will focus on how to write an effective outcome for an individual service plan. There are many more great speakers presenting including ABC for Health and Family Voices. Topic areas are broad with some focus on mental health, taking care of you, and recreation options in the area. There will be several parents co-presenting with other presenters which always adds great value and perspective to a subject.
This conference provides families and providers the information and tools they need to work together in providing coordinated community-based care.
