The Developmental Disabilities research team at the University of Kentucky requests that you complete aquestionnaire related to your experiences as a parent with a child with a disability. This questionnaire will be used for research purposes.

The purpose of our study is to determine the need and use of parent resources regarding advocating for educational services. Your responses to the attached questionnaire will inform professionals and parents about the need for these services as well as contribute to finding ways to improve the quality of comprehensive treatment for high school students.

We will greatly appreciate your completion of the questionnaire and should only require approximately 10 minutes of your time. Your involvement in the study is voluntary, your identity will not be associated with your responses, and your participation will remain confidential.
