Last Chance to Complete Self-Determination Survey

Self-Advocates: Your Input is Very Important!
Are you an included and valued member of your community?  How much control do you feel you have over your own life?

The Wisconsin Board for People with Developmental Disabilities (BPDD) would like to know how involved you are in your community, and how much control you have in your life. BPDD works  to ensure people with developmental disabilities have the opportunity to be active, valued members of the community and feel empowered to make decisions about their own lives. 

BPDD’s  Self-Determination Survey will help get a better understanding of how involved you are in your community, how valued you feel, how much control you have over your life, and how BPDD can support you in these efforts.

The survey is anonymous and can be completed online. Please complete the survey by January 11, 2013.
